"Mrs Bouman-Kuiper works at Dr Ackermann’s practice. I was never addressed using the wrong pronouns or treated in a derogatory manner at the practice. However, it is clear from the practice’s anamnesis form that it was not written with a focus on trans* people. Ms Bouman-Kuiper, however, is very professional; she never addressed me using the wrong pronouns and was also open to criticism of the anamnesis form."
