"The medical history form is unfortunately not formulated in a gender-neutral way.
But the people at the reception and the doctor herself are very friendly and respectful.
I once said at reception that my name and pronouns had changed. It was immediately implemented and a note was made about the new name in the computer without comment, so that everyone in the practice has addressed me correctly without error ever since (this was before I changed my name, so my deadname was still on the insurance card, but they never pronounced it again after that). I was immediately called "Mr XY" and never had to explain myself or correct anything, despite the large practice and many different medical assistants.
At a later appointment, Mrs von Lieven congratulated me on the name change, asked whether the hormone therapy (which I had mentioned) felt good for me, then rejoiced with me and went on with business as usual. You can tell the whole practice team have experience with trans people and are very relaxed about it." / "As the practice is connected to the MVZ, they are technically and knowledge-wise up to date and are happy to give detailed advice. Due to my disability, I am often accompanied by a trusted person or a social worker and supported, for example, when making phone calls for appointments; this was also never a problem with the practice. I was allowed to freely decide who was present at the doctor's consultation and was treated at all times as an equal and respectfully."
