I have been attending the practice for a long time, even before my coming out, and cannot recall the medical history form or anything else. When I came out as non-binary, it was a bit bumpy with new pronouns, but everyone at the practice tried their best. Dr. Valan had a few more questions but also assured me that she would support me if there was anything she could help with. Later, I came out again as transfeminine and since then I have been addressed as she/her and Mrs. without any problems."
